Liability Waiver & Release

The participation agreement, assumption of risk, and release — signed before dancing (a guardian signs for minors).

This waiver is part of registering for classes, lessons, teams, or events with Smithville Old-Time Dance Club LLC. It is typically signed during registration; for a dancer under 18, a parent or legal guardian signs on their behalf.

Assumption of risk

I understand that dance and related physical activity carry inherent risks, including the risk of injury. I choose to participate voluntarily and assume those risks for myself and any minor I enroll.

Release & waiver

To the fullest extent permitted by law, I release the Club, its owners, instructors, and volunteers from claims for injury or loss arising from ordinary negligence during participation, and I agree to follow safety instructions.

Medical

I confirm the participant is able to take part, and I authorize the Club to seek emergency medical care if needed. I am responsible for related costs.

Guardian consent (minors)

If the participant is under 18, I am their parent or legal guardian and I agree to this waiver on their behalf.

Note

Enforceability of releases varies and is limited for minors in some circumstances. This draft must be reviewed by an attorney before use as a signed agreement.
